While over-the-counter topical minoxidil is widely available, many patients experience better results — or improved tolerability — with compounded oral minoxidil capsules at low doses, or with a custom topical formula that adjusts the concentration or combines minoxidil with other active ingredients such as finasteride or tretinoin.
Compounded minoxidil preparations also eliminate the propylene glycol found in many commercial topical products, which is a common source of scalp irritation and contact dermatitis in sensitive patients.
Acne Treatments
Commercial acne medications are available in limited concentrations and formulations. For patients with persistent or treatment-resistant acne, compounding allows a dermatologist to prescribe a preparation with the exact concentration of active ingredients — such as tretinoin, niacinamide, azelaic acid, clindamycin, or ivermectin — in a vehicle appropriate for their skin type. Hyperpigmentation and Skin Brightening
Conditions such as melasma, post-inflammatory hyperpigmentation, and sun damage are frequently treated with combinations of active ingredients including hydroquinone, kojic acid, tretinoin, and azelaic acid. Compounding allows these ingredients to be combined in a single preparation at the concentrations a dermatologist prescribes — often more effective and more convenient than using multiple commercial products simultaneously.
